Job Release Form

In the event it becomes necessary to change jobs within the semester, the student must print a Job Release Form. This form must be completed not only by the student, but also by the supervisor and business administrator of the department the student is leaving. The business administrator will then fax the completed form to the Student Employment Office. Failure to comply with this procedure may result in a delay of paycheck at the new department.
